D&D 5e Conditions Cheat Sheet

One-page printable D&D 5e conditions reference. Every condition with its rules text — blinded, charmed, frightened, grappled, incapacitated, invisible, paralyzed, petrified, poisoned, prone, restrained, stunned, unconscious, plus exhaustion.

Conditions

BlindedCan't see; auto-fail sight checks. Attacks against have advantage; attacks from have disadvantage.
CharmedCan't attack the charmer or target them with harmful abilities. Charmer has advantage on social ability checks against the creature.
DeafenedCan't hear; auto-fail hearing checks.
FrightenedDisadvantage on attacks and ability checks while source is in line of sight; can't willingly move closer to the source.
GrappledSpeed 0 and can't benefit from speed bonuses. Ends if grappler is incapacitated or moved out of reach.
IncapacitatedCan't take actions or reactions.
InvisibleImpossible to see without magic / special sense. Heavily obscured for hiding. Attacks have advantage; attacks against have disadvantage.
ParalyzedIncapacitated, can't move/speak, auto-fail STR and DEX saves. Attacks have advantage; melee crit at 5 ft on hit.
PetrifiedTransformed into solid inanimate substance. Incapacitated, weight x10, ages 0. Resistance to all damage.
PoisonedDisadvantage on attacks and ability checks.
ProneCrawl-only movement (or use half speed to stand). Disadvantage on attacks; attacks against from 5 ft have advantage, beyond 5 ft have disadvantage.
RestrainedSpeed 0. Disadvantage on attacks and DEX saves. Attacks against have advantage.
StunnedIncapacitated, can't move, can speak falteringly. Auto-fail STR and DEX saves. Attacks against have advantage.
UnconsciousIncapacitated, can't move/speak, unaware. Drops everything, falls prone. Auto-fail STR and DEX saves. Attacks against have advantage; melee crit at 5 ft on hit.

Exhaustion (six levels)

Level 1Disadvantage on ability checks.
Level 2Speed halved.
Level 3Disadvantage on attacks and saves.
Level 4Hit point maximum halved.
Level 5Speed reduced to 0.
Level 6Death.

Common removal

  • Lesser Restoration — blindness, deafness, paralysis, poison
  • Greater Restoration — charm, frighten, paralysis, petrification, reduce 1 level of exhaustion
  • Remove Curse — magical curses
  • Heal — blindness, deafness, disease (and 70 HP)
  • Heroes' Feast — poison, frighten
  • A long rest — 1 level of exhaustion (with food and water)
